Future Planning And Advance Care Planning: Why It Needs To Be Different For People With Dementia And Other Forms Of Cognitive Decline

This report is from the Cognitive Decline Partnership Centre at the University of Sydney.


The project reviewed current literature and resources and interviewed more than 80 practitioners from various sectors across Australia to identify key issues and successful approaches that might inform the development of a national model.

The report contains seven key findings to improve the uptake and quality of advance care planning for people with cognitive decline, and specific recommendations and actions for government, organisations and individuals.
