An online resource that provides clinicians with evidence-based information and simple strategies to minimise the risk of functional decline for older people in hospital.

This is an online resource that provides clinicians with evidence-based information and simple strategies to minimise the risk of functional decline for older people in hospital and is underpinned by the principles of person-centred care.

The topics covered are: assessment, communication and person-centred practice; cognition, continence, falls and mobility, frailty, medication, nutrition and swallowing, pain, palliative care, pressure injuries and skin tears, along with fact sheets, implementation case studies, information on clinical handover, health service initiatives and information for consumers.

The material included in Older people in hospital and the accompanying e-learning and audio visual resources are classified as informal learning opportunities. Practitioners can choose to keep a record of the number of hours spent on the site, as self-directed and internet-based learning, which contributes to the informal learning component of Continuing Professional Development required for ongoing accreditation.

The resource was developed by the National Ageing Research Institute, Victorian health services, The Clinical Leadership Group On Care Of Older People In Hospital and Victorian Health Services, with input from other subject matter experts.
